What is the Sickle Cell Beacon Society?

Under North Central Family Medicine (NCFM) management, the Sickle Cell Beacon Society is dedicated to supporting individuals living with sickle cell disease (SCD) through a structured, person-centered support model.

The Beacon Society serves two primary purposes for participants:


1.
To offer ongoing support to help manage the emotional and physical challenges of SCD.

2.
To provide information about and access to healthcare services available through NCFM during health center visits.

Upcoming Beacon Society Sessions

The Beacon Society hosts two in-person sessions each month. Refer to the cards below for additional details.

Session #1: Building a Patient-Centered Foundation 


Session Objective:
 

Build a foundation of trust and mutual understanding between participants and the support group leader, fostering personalized and effective support.

Key Activities:

1. Patient Profile Insights - Discover the unique backgrounds of each patient, including medical history and personal experiences, to build empathy and understanding for those living with SCD.

2. Personalized Needs Assessment - Dive into tailored assessments to identify specific needs such as pain management, emotional support, and social connections for comprehensive care.

3. Health and Lifestyle Goal Planning - Partner with patients to set achievable goals that support a balanced and proactive approach to managing SCD.

Expected Outcome:

Leave with a clear understanding of your genetic and blood profiles, ready to manage your health proactively and advocate in medical settings.


Session #2: Knowledge Empowerment - Understanding Genotype and Blood Type


Session Objective:


Empower participants with knowledge about their genotype and blood type to enhance health outcomes and strengthen their self-advocacy skills.

Key Activities:

1. Understanding Sickle Cell Genotypes - Dive into the different sickle cell genotypes (e.g., SS, SC) and learn how each may uniquely impact health and treatment needs.

2. Blood Type Essentials - Discover the importance of knowing your blood type, particularly in managing sickle cell crises and ensuring safe transfusions.

3. Building Self-Advocacy Skills - Gain the knowledge and confidence to effectively communicate your genotype and blood type with healthcare providers for informed, responsive care.

Expected Outcome:

Establish strong foundational relationships and gather insights on each patient’s unique needs to deliver meaningful support in future sessions.
